Is it ADHD or Bi-Polar Disorder'
Bi-Polar Disorder, or Manic Depression, is characterized by mood swings, sometimes extreme, ranging from depressed to normal moods, or from depressed to manic episodes. Manic behaviors are often very similar to "hyperactive" behaviors, including motor restlessness, irritability, temper outbursts, sleeping less, or having higher levels of energy. It is rare. But it does happen. About three times per year a child is brought to my office for an assessment for ADHD who ends up having "Early Onset Bi-Polar Disorder." These children are typically displaying very extreme behaviors, such as throwing their bicycle -- off of the roof -- at the policemen who have been called, and so on. (When I received that call I wasn't sure what to ask first. "What was he doing on the roof with his bike'" was the best I could think of. "He was going to ride off of the roof, of course." was the answer from the child's mom.) The behaviors may not be constant or consistently displayed as one might expect from an ADHD child, and may be more extreme than one would expect from ADHD. The big clue is taking a family history and seeing if either of the parents, their brothers or sisters, or their parents, have/had a bi- polar disorder. If so, then this clue will be very helpful in making a proper diagnosis. I usually refer families to UCLA for a better evaluation, but in your community you may need to rely on an evaluation by a very good psychiatrist (never go to a bad or even average psychiatrist, only good ones!). Ask about experience with bi-polar disorder in children. Telephone teaching hospitals in your area if there are any, or call your local private psychiatric hospital. Don't ask the switch board operator for a referral, instead ask for the children's unit or the adolescent unit. Once transferred there, ask the unit nurse who they would recommend ("If it were your child, who would you take your child to see'") The nurses know. For example, making unwise choices on sex, drinking, or driving.4. children do not own their own behavior or consequences, which can lead toa life of turmoil.5. children may allow others to think for them.6. They may allow someone else to define what his or her abilities will be.This denies their maximum potential.7. When someone has weak boundaries they pick up other's feelings.8. Weak boundaries may make it hard to tell where we end and another personbegins. What is a parent to do? Many times we hinder our children from developingboundaries. Realize we must teach our children boundaries; they are notborn with them. Here are a few suggestions to help develop boundaries.1. Recognize and respect the child's boundaries. For example, knock ontheir closed bedroom door instead of just walking in.2. Set our own boundaries and have consequences for crossing them.3. Avoid controlling the child.4. Give two choices; this helps our children learn decision-making skills.5. When you recognize that boundaries need to be set. Do it clearly, do itwithout anger, and use as few words as possible.6. We need to say what hurts us and what feels good.7. It may be difficult to set a boundary. You may feel afraid, ashamed, ornervous, that's okay, do it any ways.Another way to work with boundaries and children is to model these for ourchildren.1. Recognize your physical boundaries.2. You have the right to request proper treatment, for example, poorlyprepared meals in a restaurant should be sent back, ask others to smoke awayfrom your space, and ask that loud music be turned down.3. Share your opinions with your children. Allow your children theiropinions. Opinions are not right or wrong. This will help them think forthemselves. 4. Teach them how you decide on the choices you make.5. Lets own what we do and what we don't do. Take responsibility for whenthings go wrong.6. Accept your thoughts, it is who you are.7. Discover what your limits are, emotional and physical.Setting boundaries is all about taking care of ourselves. This is the firstguideline we teach in our workshops. Other benefits include:1. These children are within a realm where they feel and respond much differently than others. There has been much focus on trying to eliminate certain behaviors or to evoke particular responses in children which actually become rote and repetitive for them without context. One of the goals in aiding these children should be in helping them find meaning. In order to do this we must be willing to not look at the child as broken, unable to respond, or even unable to communicate. These children DO communicate, however they are not always able to manipulate their senses to communicate in the typical ways of other children. As a result, they can become easily frustrated and trapped. The therapist must enter their imaginative world and learn to communicate in their language.
